For the convenience of new & prospective clients, information packets & registration forms are available for downloading. Please note that specific registration forms and Client Information Guides are available for Adults & Couples, Children (ages 12 and under), and for Teens (ages 13-17). New clients will need to download a registration form, a Client Information Guide, and a copy of "Considering Seeking Help from a Psychologist". Please bring your registration form, and the signature pages from the Client Guide to your first appointment.
